2. Expected benefits

3. Possible risks and limitations
- A clinician cannot perform a full physical examination remotely, which may limit diagnosis.
- Poor video, audio or internet quality may delay or interrupt your visit.
- Despite strong security measures, electronic communications carry a small risk of unauthorized access.
- The clinician may decide telehealth is not appropriate and refer you to in-person, urgent or emergency care.
- Some medications, including certain controlled substances, may not be prescribed through telehealth.
4. Not for emergencies
Telehealth is not for medical emergencies. If you think you are having an emergency, such as chest pain, trouble breathing, stroke symptoms or thoughts of harming yourself, call 911 or your local emergency number, or go to the nearest emergency room right away.
5. Your responsibilities
- Give complete and accurate information about your health, medications and allergies.
- Tell the clinician your true physical location at the time of the visit, because licensing rules depend on it.
- Join from a private place, and follow the treatment plan or seek further care when advised.
- Do not record visits without the clinician’s permission.
6. Faith-based and naturopathic care
Faith-based networks may include lifestyle and spiritual wellness guidance alongside conventional medicine. Naturopathic services are provided by licensed naturopathic doctors where permitted by law; their scope of practice varies by state and country, and naturopathic care is not a substitute for conventional medical treatment of serious conditions. You choose which type of care you receive.
7. Privacy of your information

8. Your rights

9. Minors
A parent or legal guardian must give consent for a patient under 18, unless local law allows the minor to consent on their own. A parent or guardian may need to be present during the visit.
10. Your consent
By requesting or taking part in a telehealth visit through MyTeleHealth.site, you confirm that you have read and understood this consent, that you have had the chance to ask questions, and that you agree to receive care by telehealth. The individual care network or clinician may also ask you to sign their own consent form.
