1. Who this notice applies to
This notice adds to our Privacy Policy and applies only to residents of California. It is provided under the California Consumer Privacy Act, as amended by the California Privacy Rights Act (together, the “CCPA”).
Medical information governed by HIPAA or the California Confidentiality of Medical Information Act (CMIA) is exempt from parts of the CCPA. That information is protected as described in our HIPAA Notice of Privacy Practices.
2. Personal information we collect
In the past 12 months we have collected the following categories of personal information from you directly, from your devices and from the care networks that treat you:

We keep each category only as long as needed for these purposes or as required by law.
3. No sale or sharing
eFiberNet Inc. does not sell your personal information and does not share it for cross-context behavioral advertising, and has not done so in the past 12 months. We do not knowingly sell or share the personal information of consumers under 16. We use sensitive personal information only for purposes permitted by the CCPA, such as providing the services you request.
4. Disclosures for business purposes
We disclose the categories above to care networks and clinicians that treat you, and to service providers for hosting, security, email and payment processing, under contracts that limit their use of the information.
5. Your California privacy rights
- Right to know what personal information we collect, use and disclose, and to get a copy of it.
- Right to delete personal information we collected from you, subject to legal exceptions such as medical-record retention.
- Right to correct inaccurate personal information.
- Right to opt out of sale or sharing. We do not sell or share, but you may still submit a request.
- Right to limit the use of sensitive personal information to permitted purposes.
- Right to non-discrimination for exercising any of these rights.
6. How to submit a request
Send a request through our contact form with the subject “California privacy request”. We will confirm receipt within 10 business days and respond within 45 days. If we need more time, up to another 45 days, we will tell you why.
To protect you, we will verify your identity by matching the information you give us with the information on file. You may use an authorized agent; we will ask for proof of the agent’s authority and may ask you to verify your identity directly.
We honor Global Privacy Control (GPC) browser signals as a valid opt-out request.
7. "Shine the Light" and CMIA
Under California Civil Code § 1798.83, California residents may ask whether we disclosed personal information to third parties for their direct marketing. We do not make such disclosures. Under the CMIA, we will not disclose your medical information without your authorization except as permitted by law.
